Rural Health Initiative is a community supported program focused on improving and sustaining the health and safety of the farming community by making house calls for preventative health screenings. Outreach coordinators are trained health care professionals who understand the farming lifestyle. (We are not insurance , or government funded). We provide on site health screening for blood pressure, cholesterol, and glucose (diabetes). We provide health coaching on a variety of topics and give referral information to local community resources. We service Shawano, Outagamie, and Waupaca counties and to anyone who contributes to agriculture. Common participants include the adults of family farms, dairy farms of any size, hobby and crop farms, Amish and agribusiness. We have Spanish interpreters when needed. This is about convenience by coming to the farm, and we ask that you help cover a small portion of the cost (20%) if possible. It is easy to make an appointment- simply call Rhonda at 715-524-1488 or Dawn at 920-639-1183 and we'll set up a time that works for you. It's that easy- 1 call away from service.
